workshop submission form-- downloadable form?

Hi all,

A question that came out of our meeting this evening: Is a download-able (and then printable) workshop submission form available online yet? (I am not yet registered under an organization, and so cannot check). If not, we would like folks to be able to download and then print out the workshop submission form, to be mailed in. Kiran, can you and the Program WG create a paper form based off of the online form? I don't think there's a process for creating downloadable forms other than whoever needs it does it. I created the downloadable forms for registration and scholarship as .pdf files (at  http://www.ussf2010.org/register). The Disability Justice Committee asks that forms also be created in 14pt, sans serif font, saved as .doc files and sent to access@…. I'd suggest following up with Gwen Gaines (gwendolyngaines@…) at the USSF Detroit office to make sure they can get printed and made available for walk-in proposals. Please let me know if I can help.

We're working on the paper form now. I don't think we had realized how much needed to be changed to create the paper form ;)

Is there a group of folks that can do the layout/design/make it look sleek and add the USSF banner to the form once we've finished with the wording? I remember the Consulta paper form had the USSF banner and beautiful layout, but don't know which group headed that piece of it up.

Ok, here are two documents that contain all the text the Program and Culture working group would like on the printable forms. The issue: they are not layed out well. Mark had mentioned Design Action Collective potentially doing the layout, so we decided I should just send them along to speed up the process of getting them online. At the very least, they could do to have the USSF banner put on them. Let me know if there is any more action needed from Program and Culture.

hi all,

so first off, i just wanted to check in on behalf of the program and culture group so see where this process is at and if you need anything from us.

second, in addition to just getting pdf versions of the form up so that folks can print them out and mail them in to us, the group wanted to stress that they want the pdf versions up so that even the folks who are planning on submitting the forms online can download the forms and therefore be able to see all the questions in advance, take them to their organizing groups, and be prepared with well-prepared and thought out answers to the questions we're asking when they go to fill the forms out online.

third, sylvia: i've been in touch with disability access. thanks for that heads up. we have not yet followed up with gwen. i'll see if someone can do that at our next meeting, on monday.

Hi Kiran - sorry I don't have a progress report - but at least for now I'm putting it into milestone 4 critical to ensure it's addressed with our most urgent issues.
