Potential USSF endorsers must submit info about themselves

Description

For the endorser fields, it looks like we'll need:
* Organization or individual's name
* Contact person
* contact email
* Org or indiv website

Should this be a cck type, like testimonials, or a webform?

The information will be vetted for spam (we should also use captcha) by AMB, accepted, and then posted on the site in a list.

I think cck would be better so we have better options for displaying (even though it's more work to set up).
